Our liaisons assist community partners, prospective clients and their families, in planning for safe discharge from healthcare facilities. They educate on in-home service options and help with the process to obtain care.

Working closely with their Angels’ teammates, community partners, clients and their families, the intake department coordinates care from the first phone call to admission.

Coordinators are responsible for scheduling clients and caregivers in order to provide the highest quality service, with an emphasis on creating extraordinary relationships.

Our Client Service Managers are responsible for overseeing the office operations, and collaborating with their support staff, clinical team and community partners to guarantee clients and patients receive the quality care they deserve.

Our Field Service Managers are in charge of the coordination and administration of all caregiver trainings. They provide our initial in-home assessments and support individual client needs through additional education and quality oversight.

We help families just like yours with a tailored approach to care, addressing the needs of your loved ones, specifically for your loved one.

We work closely with several insurance partners so we can live by the mission that all people deserve quality care.

Some of our patients require specialty care at home. Angels On Call is proud to offer the area’s top physicians for in home care services.

Skilled Nursing Facility – When your family member is ready to go home our Angels On Call Home Health team will provide your loved one with continued clinical services, nursing, PT, OT and more. Ask the case manager or social worker to call our office,

Primary Care Physician – Nobody knows you better than your primary care doctor. We can coordinate in home care for you or your loved one and work together with your doctor to facilitate the appropriate plan of care.

Our social workers are able to help you with additional services that you may be eligible for.

Once you are ready to be discharged from the hospital we will work together with the case manager or social worker to design a plan of care at home that is suited for you. Just ask your case manager or social worker to make the referral to us.
