Must be a Michigan resident with active Medicaid
(Don’t have Medicaid? Angels On Call will guide you through the application process) - Require assistance with at least 1 ADL (Activity of Daily Living) due to medical condition, disability, or cognitive impairment
- Live at home (individuals living in assisted living or group home settings may qualify too)
Caregiver Eligibility
- Must be at least 18 years old
- Must pass a criminal history screening before providing services
- Must be registered in the CHAMPS system
- Can be a family member, friend, neighbor, or someone else the member trusts
